A far too common story

When my mom called me to to ask for help finding a physician who could help my dad to get his diabetes health under control, I quickly learned that while accessing the right kind of care was possible, it was far from easy. That’s because few doctors focus on diabetes & metabolic health and the ones who do, do not live the challenge at home like you do and were not trained in nutrition in medical school. As I researched the problem, I realized that fixing Kansas’ diabetes and metabolic health crisis couldn’t be done by just prescribing more medications alone. We needed to use technology to help the best care reach more patients and a medical provider who understands their challenges.

I founded Evolve Diabetes LLC. because traditional diabetes appointments in the doctor’s office every 3-6 months and that focus on medications and labs alone don’t work—for diabetes or for long-term health. That’s because diabetes doesn’t only reflect numbers on your lab work. It reflects on the 180 microdecisions that you make everyday at home.

Evolve Diabetes LLC. is the modern, medical solution provided by a diabetes clinician who lives diabetes everyday just like you do and gives you the professional and personal tools to control of both your blood sugars and your health, and who can harness the many factors that make up your unique biology, your lifestyle and your personality. We combine what works—incorporating the  proven science of continuous glucose monitoring and medical appointments to personalize your plan for medication, food, sleep exercise and emotional health. Evolve Diabetes LLC was founded under the supervising role of a local primary care physician who understands that patients cannot get all the help that they need from one appointment every three months in the regular doctor’s office.
